Ready-made medical forms for iPad and Android

Formotus offers a selection of medical forms that are ready to use immediately by anyone with a Formotus subscription. These forms can capture signatures and diagrams as well as multiple-choice and free-form text. The forms can be emailed to any address and they arrive as two email attachments: one an image of the completed form for filing or printing, the other an XML data file that can be opened in Excel, for example. Currently available forms include:

• Health Treatment Authorization – Includes signature capture
• Health Pain Questionnaire – Includes human body pain diagram and pain scale faces
