Forgotten Password

A password was emailed to the account you use to login to Formotus forms on your mobile device. Click here to request a new password in email.  If you do not receive an email, please contact your administrator to assist in resetting your password.

About Being Connected

When a device is connected to Formotus, all Formotus features and functions are available. Connected means:

  1. The device has data access to the Internet; and
  2. The Formotus forms program is in communication with the Formotus server.

Not Connected and Working Offline

The “Offline” message appears when the program is not in communication with the Formotus server, usually for lack of Internet availability. You can work offline, provided you have recently logged in online with the Save Password option selected. You can work offline for a limited number of days before you will be required to connect and log in again. As the time limit approaches you will receive alerts at the bottom of the screen.

If you are offline and you tap Refresh, the program will attempt to connect. Any action that requires a connection, such as downloading a form, will attempt to connect.

Features available working offline:

  • Open the program and use saved credentials to log in
  • Open an installed form and save multiple drafts of it
  • Submit multiple instances of a filled form to the Outbox

Features NOT available working offline:

The following actions will cause the Formotus program to connect to the Internet if possible, or will fail if there is no connectivity.

  • Install a new or updated form already known to be available
  • See in My Forms that a new or updated form has become available
  • Send forms from the Outbox
  • Check for program updates from the Settings page
